Fax the completed registration form to Aly Pennucci 612-625-3513 by Friday, August 31.
Please note, while the events are aimed at Blue Cross and Blue Shield of Minnesota community funding recipients, there are a limited number of additional seats available for others interested in attending the workshop portion of the events. Registrations will be considered on a space available basis.
